Workers' accident compensation insurance is a system that provides necessary insurance benefits for workers and their bereaved families in the event of injury, illness, disability, or death of workers due to work-related reasons or commuting.
This system is based on the Workers' Accident Compensation Insurance Law (Workers' Accident Compensation Insurance Law). Since our clinic is designated as a "designated medical institution for workers' accident compensation insurance," we are eligible to receive medical treatment benefits for work-related and commuting disasters.
In this case, there is no co-payment as the full cost of treatment is paid.

Please bring one of the following forms if you are receiving treatment under workers' compensation insurance.
If you are receiving treatment at our clinic for the first time
For commuting disasters: Form No. 16-3 (Application for medical treatment benefits as medical treatment benefits)
For work-related disasters: Form No. 5 (Application for medical treatment benefits as medical treatment compensation benefits)
When changing medical institutions for treatment due to transfer, surgery, etc.
For commuting disasters: Form No. 16-4 (Notification of (change in) designated hospital, etc. where medical treatment benefits are provided)
For work-related disasters: Form No. 6 (Notification of (change in) designated hospitals, etc. receiving medical treatment benefits as medical treatment compensation benefits)
*All treatment costs are to be paid in full at the counter until the necessary documents are submitted to the clinic.
As soon as the documents are submitted and registered, we will process the refund of your co-payment.
